Identification Requirements:
For all piercing services, valid identification is required:
For adults (18+): State-issued driver's license, state-issued ID card, resident alien card, military ID, or passport.
For minors: Birth certificate and school ID. Minors must be accompanied by a parent or legal guardian with the necessary documentation. If you are the parent of a minor, but you and the minor have different last names, we need documentation showing why your names are different (i.e. divorce, name change, etc.) or some other legal documentation demonstrating guardianship.
